11 Partner ACOs generated $43.6M in Shared Savings Revenue in Performance Year 2016 | December 2017
The amount of shared savings generated by 11 ACOs represents a 9.5 percent increase over what the CHS-affiliated ACOs generated last year. Six of the ACOs are Track 2 MSSP ACOs in which providers take on more financial risk. Read the announcement.

Providers in the Accountable Care Coalition of Chesapeake use a variety of tools to coordinate patient care, supporting the goal of improved healthcare outcomes at lower costs. CHS Physicians Launch Pilot with Community Pharmacists to Improve Health Outcomes | December 2016
Primary Care Physicians in the Accountable Care Coalition of the Tri-Counties will share patient data with community pharmacists to drive better patient health outcomes and improve preventive care. The physicians and pharmacists will engage in frequent communication to offer evaluation of polypharmacy risk and predictive outcomes among beneficiaries in the ACO. Read the announcement.
Universal American ACOs and Fresenius Medical Care Partner to Improve Health of End Stage Renal Disease Patients | November 2016
Collaborative Health Systems (CHS), a Universal American company, and Fresenius Medical Care announced an agreement to provide integrated health care management for patients with renal disease. Fresenius is North America’s largest provider of kidney care services. Read the announcement.
Five partner ACOs selected to "100 ACOs to Know 2016" | September 2016
The ACOs include Accountable Care Coalition of North Texas, Accountable Care Coalition of Southeast Texas, Accountable Care Coalition of Southeast Wisconsin, Maryland Collaborative Care, and Virginia Collaborative Care. ACOs were selected by Becker’s Hospital Review based on excellence in quality, achievements in driving down cost, and ability to grow and take on risk. Read the announcement.
Continued Improvements in Quality and Cost of Healthcare | September 2016
Ten CHS accountable care organizations (ACOs) formed in partnership with primary care physicians earned $40 million in shared savings as part of the Medicare Shared Savings Program (MSSP) in 2015. The amount of shared savings represents a 48% increase over last year and reflects a corresponding improvement in quality scores for all of our ACOs with start dates prior to 2015. A New Generation of Value Based Care | January 2016
Accountable Care Coalition of Southeast Texas, in partnership with CHS, was chosen by the Centers for Medicare and Medicaid Services (CMS) to participate in the Next Generation ACO Model. The new model encourages providers to assume greater risk and reward in coordinating the healthcare of Medicare Fee-For-Service beneficiaries. Read the announcement.
